Title: Community Inclusion Facilitator

Hours: 9:00-3:00 (Monday-Friday)

Location: Livermore

Rate: Up to $26/hour

Requirements:

  • High School Diploma or GED 

  • Current valid driver’s license

  • Ability to adjust auto insurance to company’s requirements

  • Clean DMV record

  • Reliable transportation

  • Ability to transport up to four adults in personal vehicle

  • Ability to drive eight seater company vehicle

  • Ability to pass LiveScan, health screening, and TB test

Essential Functions:

  • Implement program curricula

  • Provide direct supervision and support for each participant

  • Directly deliver individual and group learning experiences 

  • Provide opportunities for participants to make choices, participate, and learn 

  • Maintain data collection regarding participants progress

  • Participate in participant assessment, planning and evaluation process

  • Provide basic self-help skills, daily living skills, and adaptive living skills

  • Assist with researching community activities, events and destinations. Planning trips to desired locations, and attending outings with participants. 

  • Act as an appropriate role model for participants.

  • Be flexible in site assignment, as job duties are to be carried out in different community settings

  • Act as an advocate to promote and protect the rights of individuals with intellectual and developmental disabilities; Ensure individuals are treated with dignity and respect; Support participants in advocacy and self-advocacy actions.
